Medical Coverage
Palisades pays 90% of premium and 100% of in-network preventative care. Two plan choices are available: membership in a Preferred Provider Plan (PPO) or an Exclusive Provider Plan (EPO). The PPO plan gives you a national network, the choice of whether or not to choose an in-network provider, and the freedom to seek care out-of-network at slightly higher costs with no referral necessary. The EPO plan also gives you a national network while choosing an in-network provider only, at slightly lower costs with no referral necessary.
Dental Coverage
Two choices are available with the largest group of PPO providers in the state. Employees also have a choice of going out of network with higher out-of-pocket expenses. The basic plan has an annual maximum of $1,000 and the buy-up plan has an annual maximum of $1,500. Both plans have orthodontia coverage.
Vision Care Coverage
The plan covers periodic exams, eyeglass lenses and frames, or contact lenses, up to a maximum amount per year.
